Social Worker- Independent Living, Falcons Landing- Life Plan Community.

Falcons Landing is looking for an experienced individual with a Master of Social Work degree or a Licensed Clinical Social Worker to join the Department of Community Life in Independent Living at our vibrant Life Plan Community in Potomac Falls, Virginia.

The successful candidate will provide a full range of social services to independent living residents and their families and duties will include:

Counsel with residents; offer social work interventions; conduct support groups; conduct family conferences; serve on the independent living interdisciplinary team; provide information on resident rights, advance directives, community resources; member of the wellness education team; and adheres to Virginia Department of Social Services mandatory reporting guidelines.

Candidate Must Have:

• Master’s degree (MSW) from an accredited college or university or Licensed Clinical Social Worker (LCSW) in the Commonwealth of Virginia.
• Three to five years related experience with at least three years’ experience working with the senior population.
• Administrative ability and computer proficiency required.
• Life Plan Community (CCRC) experience preferred.
